November 22, 1939. Place of birth is listed as Broad Street Hospital, New
York, NY, but very likely Florida or Delaware.
Birthmother listed as Ruth Haverman, born in New York City, 23 at time
of birth.
Birthmother's surrender paper has proven to be a forgery.

certificate, naming my adoptive parents as my birth parents. It also states
that I was born in Phillipsburg, Kansas, but my adoptive parents tell me
they picked me up from Bryan Memorial Hospital in Lincoln, Nebraska.
