There is a short form that your doctor must fill complete. It is a very simple form and takes only a few minutes. Insurance information if you have any, your income, diagnosis, allergies etc. If you have insurance they will request a letter from them stating the following: Why they will not cover this prescription, ie. maximum has been reached or no coverage for this medicine. By knowing they will need this you can get a letter ahead of time and send it when you first apply. I don't know about the others but You must sign for the receipt of medicine. You have to send your prescription to them each month in the envelope provided about one week before you are out. Purdue sends them FedEx overnight. You MUST send your hand written script. For Ohio ODHS members you will need a letter stating why your medication is not covered by them.
