Do your hospital have guidelines for use of amphotericin B lipid complex (ABLC)? (Health Sciences Centre, Winnipeg)

Does your hospital have a protocol for ketamine IM/IV/PO use in Emergency for minor procedures? (Children's Hospital of Eastern Ontario)

Do you have a treatment guideline for UTIs which specifies: duration of treatment, when to convert from IV to PO, and what route to use when initiating treatment? (Children's Hospital of Eastern Ontario)
