INDICATORS OF REDUCED ABILITY OF CLIENTS TO CARE FOR THEMSELVES LEADING TO INVOLVEMENT OF SOCIAL SERVICES


SELF CARE

e.g. eating, bathing, use of toilet etc.

MENTAL HEALTH

e.g. memory, behaviour, emotional state, orientation, reaction to disability / illness.

COMMUNICATION

e.g. understanding and conversation which impedes ability to advocate for themselves.

LIVING CONDITIONS / DOMESTIC CARE

e.g. care of home, domestic safety, shopping, food preparation, laundry etc.

ACCOMMODATION

eg. where disability brings into question the appropriateness of the accommodation  and where there may be a requirement for appliances / adaptations.

FINANCE

e.g. where inability to manage limits the individuals ability to care for himself /  herself.

ABILITY TO USE COMMUNITY RESOURCES

e.g. use community facilities, public transport, where the individuals illness /  disability means there is an element of risk.

INFORMAL SUPPORT SITUATION (IE. CARERS)

e.g. Access to help, level of support and continuation of support where this is non- existent, or at risk of collapse etc.

PHYSICAL HEALTH AND MOBILITY

e.g. day and night continence, mobility, sensory loss, where there has been a  significant change which limits the individuals ability to care for himself / herself.
