Medical School Application Worksheet

The following file is a worksheet for the anal as well as the disorganized premed student.  It is designed to help you through the application process step by step, ensuring you haven't neglected any of the necessary components of the process.  The form is based on work done by an anonymous past Hunter College premed who gave it to Francoise from the Pre-Professional office for all to use.  I modified it a little to include info I found out to be important.  I also edited it to make it more presentable (well, taste is a matter of opinion...) and converted it to pdf (Portable Document Format).

To use it, print one copy for each med school you are applying to and fill in as you go through the application process.
