A $5.00 a month discount will be given to each member of immediate family. |

All cheques and post dated cheques are to be made out to "Kingston Tsuruoka Karate", dated for the 1st of every month, and handed in within one week after beginning training. |

If you must stop training all remaining cheques will be returned. |

A charge of $25.00 will be applied to all NSF cheques. |
