Tuberculosis
Clinical presentation:
22 year old male from the Middle East with gradual onset of cough, night sweats, vomiting and weight loss.

Features in Image
There is a density which enlarges the left hilum and extends into the left upper zone, blurring the margin of the left hilum. It occupies the site of the apico-posterior segment of the left upper lobe, but has a more complex appearance inferiorly in the lateral view. The oblique inferior margin is presumably the greater fissure. There is a lobulated left para-tracheal density. There is patchy perihilar consolidation in the slightly shrunken left upper lobe with particular collapse consolidation of the apico-posterior segment.
General Features in T.B.
Primary T.B. shows consolidation and lymphadenopathy, pleural effusion and can occur in any location. Secondary T.B. (reactivation) shows consolidation without lymphadenopathy, a posterior apical location, and fibrosis and calcification on healing. Cavitation may occur. Bronchogenic spread is seen with mottled small opacities.
