By signing this form I agree to the following:
I will spay/neuter this kitten before it is able to reproduce
I will provide adequate medical care for this animal.
I will provide adequate shelter, food, water, and love for this animal.
I will NOT use this animal for animal testing or breeding purposes.
In the event I move or am no longer able to care for the animal I will make every effort 
to place it in a good home before taking it to a rescue or shelter such as rabies animal control 
or the humane society. I WILL NOT LET IT GO “FREE” ON THE STREETS.
By signing this form I understand I cannot take legal action against Cassandra Zielske 
or her immediate family members for any reason connected to this animal or it’s adoption.
Cassandra Zielske is NOT responsible for this animal once I have adopted it. 
I have read, signed, and understand the return policy Cassandra has given to me.
