HIV INFECTION

The primary immune defect in HIV is the decline in CD4 cell number and function. Evidence of the latter includes decrease in delayed type hypersensitivity (DTH) to recall antigens, decreases in lymphoproliferative responses (LPR), and decreased production of TH1 cytokines (e.g. IL-2, IFN gamma, and IL-12). At the same time, uncontrolled viral replication results in chronic immune activation as measured by increased numbers of CD38 and HLA-DR expressing cells and increased production of pro-inflammatory cytokines (e.g. TNF, IL-1). Perturbations in CD4 and CD8 T-cell repertoires are also seen, and may reflect clonal deletion or clonal exhaustion as a result of HIV infection.

There are decrease in the total CD4 count, including memory and naive CD4 cells. Initial increase in CD8 cells, primarily the CD38+ and HLA-DR+ cells, are seen early after HIV infection, but over time, decrease in naive as well as memory CD8 cells ultimately occur.

REGNERATIVE FAILURE OF BONE MARROW AND THYMUS

The absolute number of mature T cells-those primed to recognize pathogens like HIV is determined by their production rate. In T-cell dynamics, we call the production rate of circulating CD4 T cells ks and the destruction rate kd. At steady state, when the number of circulating cells remains constant, ks must equal kd. HIV infection disturbs that balance in a way that reduces the number of CD4 cells. The question is why.

(Note: That Glucocorticoids rescue CD4+ T lymphocytes from activation-induced apoptosis by HIV-1 and this brings back balance.)

Researchers have proposed several models to explain drops in CD4 cell numbers. The two models that have received the most attention might be seen as polar opposites, but they may be better perceived as a continuum. The so-called high turnover model rests on the premise that destruction of CD4 cells accelerates during HIV infection. According to this thinking, the number of mature T cells drops because HIV speeds up the rate at which they are destroyed. The other alternate possibility that we call regenerative failure. This model envisions that the supply of mature T cells dwindles because HIV decreases their rate of production. HIV lowers CD4 counts by increasing destruction of T-cell progenitors. Both models point to HIV as the cause of T-cell declines and to HAART as the agent of T-cell rebounds. In fact, both models may be correct, but at different times in the course of HIV disease. In early disease there may be accelerated destruction of mature cells and a compensatory increase in their production from progenitor cells. Later, the T-cell count may drop precisely because the progenitor cells are finally destroyed, thereby creating a state of regenerative failure.

Evidence for regenerative failure can be found in two central sites of hematopoiesis-bone marrow and thymus. Studies have shown that the cytopenias common in HIV disease reflect disruption of the hematolymphoid microenvironment, including bone marrow and thymus, in association with abnormal expression of cytokines known to be important for hematopoiesis. This early work suggested that HIV may affect progenitor cells both directly and indirectly.

Opportunistic infections can speed up the process of T-cell destruction and increase HIV virus replication.

WHAT CAUSES AIDS?

HIV is a slow progressive disease but it can be increased in its progression to AIDS.

One HIV activist on the internet writes that Herpes virus HHV-6A is a co-factor necessary for the progression to AIDS.

This theory is not yet proven. I have noted that 70 percent of cats have herpes. If the cat also has FIV it is likely the percent is even higher. Since it is difficult at times to detect herpes and the cat has FIV, it can be assumed that herpes is also there.

This virus will speed the progression to AIDS. When treating FIV, also treat for Herpes. What we can do in treating this disease is make a slow progressing disease even slower. The use of Colostrum which contains transfer factor (cytokines) may be a help in treating this disease. It will inhibit HHV-6A.

There is the belief that only HIV progresses to AIDS. HHV-6A uses the same CD4 receptor as HIV and can destroy that cell. HHV-6A can progress to AIDS without HIV infection. HHV-6A is actively replicating and destroying lymphoid tissue, it is probably the primary cause of swollen lymph nodes. This disease should be of equal concern as FIV infection. Both HIV and HHV-6A together make for a deadly combination. What was originally identified as harmless herpes is now turning out not to be so harmless.

ANOTHER CO-FACTOR TO PROGRESSING AIDS

Mycoplasma

These are now considered important emerging pathogens in causing chronic diseases and may be important cofactors in some illnesses, including AIDS and other immunosuppressive diseases.

Mycoplasma species are a fastidious bacteria lacking a cell wall. They do not survive for more than a few hours outside the host and are vulnerable to disinfectants. Most strains are sensitive to antibiotics.

The earliest signs are acute swelling and redness of the conjunctiva in one or both eyes. Severe sneezing and nasal discharge with unilateral conjunctivitis indicates herpes virus infection. Mycoplasma eye infections can be treated with nonsteriod eye ointments-Tetracycline.

It has been documented that effective treatment of mycoplasma contamination in cell lines with enrofloxacin (Baytril).

Grapefruit Seed can be used for treating mycoplasma. It is used in hospitals as a disinfectant.

COINFECTION OF BARTONELLA HENSELAE AND FIV INDUCE CLINICAL DISORDERS IN CATS

Feline Infectious Anemia (Hemobartonellosis) Feline hemobartonellosis is an infectious disease caused by a rickettsial parasite known as Hemobartonella felis. Most cases are seen in outdoor cats and transmission is likely accomplished by fighting and biting. It can also be spread by blood-sucking parasites such as fleas and ticks. What results is a loss of red blood cells from the system (anemia).

This disease can only be confirmed by actually visualizing the parasite, with a microscope, on a blood smear.

Proper treatment utilizes the tetracycline group of antibiotics to control the infection, and moderate doses of prednisone to arrest the destruction of red blood cells. Treatment does not completely eliminate the parasite and a cat that has recovered from the infection remains a carrier for the rest of its life. The disease can recur with FIV infection.

Coinfection with FIV may be associated with gingivitis and lymphadenopathy in cats.

TREATMENT OF AIDS-ANEMIA

Anemia and neutropenia are common problems in patients with HIV and FIV infection, and may cause symptoms that are life-threatening. The causes of anemia in FIV disease include direct and indirect stem cell inhibition by the virus, increase peripheral destruction of red blood cells, and bone marrow suppression by various opportunistic infections.

Supportive care for anemia includes the use of erythropoietin in addition to the judicious use of red blood cell transfusion. Treatment for neutropenia includes the use of the myeloid growth factors G-CSF and GM-CSF; erythropoietin and granulocyte(macrophage) colony-stimulating factors (G-CSF) have been used in clinical trials. Other treatment factors for anemia are: vitamin B12 and folates.

Hematopoietic growth factor (rhGM-CSF and rhEPO) as a supportive therapy for FIV-infected cats.

GM-CSF increases FIV load unless used with antivirals, EPO (erythropoietin) does not increase load. EPO treated cats developed elevated WBC counts consisting of either neurophils, lympocytes, or their combination by 4 weeks post-first treatment. EPO can be used safely on FIV-infected cats

The herb Stinging Nettles may help in treating for anemia.

There are some Chinese formula for treating less life-threatening situations.

Marrow Plus

Si Wu Tang (Four Material Decoction) is a TCM formula to treat Blood Deficiency. It contains angelica, rehmannia, paeonia and cnidium. Its function in TCM is to tonify and regulate blood.

ACUTE MYELOID LEUKEMIA

Cats with respiratory toxoplasmosis often have a fever and show signs of malaise, weight loss and lack of appetite. Labored breathing becomes more pronounced as the disease progresses. Gastronintestinal signs are also common among cats with secondary toxoplasmosis. The most frequent signs are fever, weight loss, vomiting, diarrhea, mesenteric lymph node enlargment, and palpable thickening of the abdominal wall. Clinical signs resulting from central nervous system involvement include personality changes, incoordination, convulsions, unequal pupil size, blindness, plaintive crying, circling, ear twitching, and impaired swallowing. Ocular involvement can be the only sign of illness. Chronic and healed lesions can coexist in the same eye.

Diagnostic Procedures: Veterinarian should contact Colorado State University at 303 491-1274 for instructions on what samples to send, how to ship, and exact costs.

Treatment for active infection: If an animal that becomes clinically ill is diagnosed promptly and correctly, toxoplasmosis is responsive to clindamycin (25 mg/kg orally in divided dose two to three times a day for 3 weeks) or trimethoprin sulfate(120mg/kg orally twice daily for 3 weeks). Other is Tribrissen(8-15mg/lb) or Clindamycin with Pyrimethamine(1mg/kg/da) and Folic acid 5mg/da. Usnea has shown killing of toxoplasma in vitro.

HISTOPLASMOSIS

Histoplasmosis is the most frequently diagnosed systemic fungi disease in the United States. Cell-mediated immunity is critical in preventing dissemination of histoplasmosis after infection of the respiratory tract.Acute Histoplamosis: flu like illness and fever, anorexia, nonproductive cough

Diagnosis: microscropic examinations of specimens from sputum, bronchoaveolar lavage, or tissue biopsy, culture of blood and urine, bone marrow specimen.

Opportunistic infections (OIs) remain the most important complications of HIV infection, resulting in profound morbidity and mortality.

Development of an OI probably increases HIV expression and immune damage by causing immune activation. It affects HIV with significant shorter survival.

Pneumocystis Carinii Pneumonia (PCP)

Pneumocystis carinii has been classified as a fungus. It is the most common opportunistic infection.

Sites of Infection: Usually the lungs. Rarely, in extrapulmonary sites, including the lymph nodes, bone marrow, spleen, and liver.

Symptons: Fever; dry, nonproductive cough; difficulty breathing.

Diagnosis: Chest x-ray, blood tests and samples of phlegm or spit for the laboratory.

Treatement: Antiboitics (septrin or co-trimoxazole)

Prophyloxis:

Oral Co-trimoxozole (Bactrim/Septra) Daily

Dapsone and Trimethoprim Daily

Dapsone/Pyrimthamine Twice per week

Sulphadoxine/pyrimethamine (Fansidar) tab per week

GASTRIC INFECTIONS IN AIDS

The GI tract has long been recognized as a major site of AIDS-relate pathology, and up to 90% of HIV-positive patients experience GI symptoms during the course of their disease. The immune deficiencies caused by HIV allow for many different GI opportunistic infections, usually affecting the esophagus or intestine. Opportunistic infections are less common in the stomach, however, and the gastric manifestations of HIV disease have been more difficult to characterize.

The exact incidence of HIV-related gastric infections is difficult to determine, since many are asymptomatic. Bacterial, viral, fungal, and protozoan pathogens have all been reported to cause gastritis, with the likelihood of different infections dependent on an individual patient's degree of immunosuppression. Helicobacter pylori, which is present in 90% of immunocompetent patients with chronic active gastritis, continues to be extremely common in those whose CD4 cell counts are greater than 300/uL. In patients who have AIDS and CD4 cell counts below 200/uL, however, gastric inflammation and ulceration are more often secondary to cytomegalovirus (CMV) or Cryptosporidium paryum infections. Other gastric infections seen in patients with AIDS include those caused by Toxoplasma gondii, Leishmania donovani, Mycobacterium avium intracellulare complex (MAC). Treponema pallidum, Bartonella henselae, Baronilla quintana, and Cryptococcus neoformans, as well as phlegmonus gastritis. In addition, neoplasms such as Kaposi sarcoma and non-Hodgkin lymphoma may involve the stomach and present clinically with symptoms similar to those of gastric infections.

Mycobacterium Avium Complex(MAC)

Disseminated Mycobacterium avium complex(MAC) infection occurs in 30% of AIDS patients over the course of the disease, and it is the third most common opportunistic disease.

The organisms are ubiquitous in the environment-found in soil, water, and in a variety of animals and foods.

The risk of developing disseminated MAC disease is greatest for patients with CD4 counts below 50-75 cells/L and in those who have had a prioror bone marrow cultures. Recovery from other body fluids, tissue or stool culture. Often treated prior to definitive diagnosis if smear positive for Acid Fast Bacilli (AFB).

Treatment is clarithromycin and ethambutol. Treatment may be discontinued when CD4 counts rises and viral loads decreases to safe levels.

Prophylaxis initiation: Rifabutin - discontinue when CD4 counts rise and viral loads decrease to safe levels.

Garlic inhibits MAC isolates from AIDS patients. This may help with MAC some. Garlic also inhibits cytomegalovirus (CMV).

IRON METABOLISM

HIV infection is accompanied by a progressive iron loading.

Iron may increase the oxidative stress, activate nuclear factor kB (NF-kB) and directly enhance transcription of HIV.

Well known consequence of iron loading is the stimulation of the growth of many microorganisms, including AIDS-opportunists.

Use lactoferrin to counter iron loading.

NEPHROPATHY (Kidney disease)

HIVAN/FIVAN is the result of direct viral expression within the kidney.

Kidney disease is a major cause of death among FIV cats. It is caused by an immune system that is too weak to normally fight off bacteria and other infections. In fighting these diseases inflammation is created which causes oxidant damage to kidney cells and distroys them. This is know as oxidantive stress. FIV itself can cause this inflammation and stress. Once over 70% of kidneys is distroyed the cat will die

It appears as if the kidneys are a protected reservoir of HIV infection, similar to what we see in the central nervous system. In patients receiving drug treatment, we're able to reduce viral load to undetectable levels in the blood. But we're still seeing active replication in the kidneys.

Treatment improvement with triple-drug antiretroviral therapy.

The use of corticosteroids reduces inflammation. Angiotensin-converting enzyme (ACE) inhibitors are sometimes used. Captopril is ace inhibitor mostly used.

Tru-OPCs (Grape seed) has angiotensin-converting enzyme (ACE) inhibiting activity

There are also herbs such as Licorice root, Nettle and curcumin which reduce inflammation.

Omega-3 may help a little as it is a weak cytokine blocker.

Dan Shen, Rehmannia six, Burdock, Cordyceps Chinese herbs used for kidney failure.

In an article, "Antifungal Drug may be new treatment for Chronic Kidney disease". They show that reduction of the production of cortisol might slow the progression of kidney failure. DHEA is an inhibitor of cortisol and also VitalPet. This may explain why some HIV symptoms disappear when cortisol is inhibited.

INTESTINAL CANDIDA

The Candida Syndrome, more commonly known simply as Candida, is a fungal overgrowth in the gut. We all have some fungus in the gut, but when it proliferates out of control it overloads the immune system. In Candida, the normal yeast has had a explosion and changed into a parasitic fungus that penetrates the bowel wall, pucturing it with little holes. FELINE HERPETIC KERATITIS

Herpes Virus Infection in the Eye

The feline herpesvirus I (FHV-1) is the causative virus of Feline Viral Rhinotracheitis (FVR), an upper respiratory tract infection, and associated viral eye infections. It has the ability to persist in latent form in the eye making reoccurrence of infection common.

Cat may show flu like symptoms, running nose, cough, tearing eyes, ulcerated eyes, constipation, very small size, even a closed eye. This virus causes conjuctivitis (inflammation of the moveable white tissue surrounding the eye) and/or corneal ulcers. Mouth culcers occur in some patients.

Three antiviral medications are available. Idoxuridine (IDU) marketed as as Stoxil and Herplex. Adenine Arabinoside as Vira A and Triflurothymidine as Viroptic.

Treatment of feline virus conjunctivitis is non specific. Tetracycline ophthalmic ointment is indicated because of its effecacy against Chlamydia and mycoplasmal infection. Anti-vrial medications should be used for corneal involvement. Don't use steroids, as they delay re-epithelialization, prolong epithelial infection to involve the corneal stroma.

For Dry Eye, commerical tear replacement products such as Tears Naturale II, Hypoteans and Isopto-Tears are suitable. Artifical tears alone do not lubricate as well as natural tears, so additional lubricant, Lacrilube, Tear Gel or Hylashield Nite.

Some herps that are HSV-1 inhibitors may work for Feline Herpes, Olive leaf, Prunella Vulgaris, Sandalwood oil, Ganoderma lucidum, Coriolus, Resveratrol.

A doctor from Italy recommended Eucalyptus oil for herps infections of the eye. External use only.

CORNEAL ULCER

Hi, Last summer I rescued 5 kittens that had pretty serious eye problems caused by the herpes virus, and two of them had severe corneal ulcers. We tried many different meds and nothing worked. I ended up taking the kittens to a veterinary opthamologist and he prescribed IDU drops w/gentocin. They are antiviral drops and the kittens showed immediate improvement. I think you should give the vet a call and ask if you can try these, and the sooner the better (like tomorrow). The fact that this problem is taking so long to resolve leads me to believe that this is viral, and if the proper treatment isn't found soon, loss of the sight in the affected eye is a real possibility. One of the kittens lost 75% of the vision in one eye because we didn't get the appropriate medication soon enough. I hope this helps.

IDU: Idoxuridine is a substituted pyrimidine analog that was the first antiviral agent to be approved. It is used topically in the treatment of herpes keratitis (0.1% solution), but because of its lack of selectivity it is too toxic for systemic administration.

I resolved our cats eye problem long ago with human fibronectin. We don't think ours was herpes but kerratitis sicca or neuroparalytic keratopathy.

in 2 mg freeze dried powder on DRY ICE. has to be kept at minus 20!

You reconstitute the powder by adding something like normal saline. I used 2ml but more dilution is reckoned possible depending on your purpose. How far you can dilute it and still get results is unknown. This product is not licenced for eye use! Its just another thing they discovered it is useful for. Promega are helpful but at 85 US for 2 mg its expensive. But what price a new eye??? Saved our critters eye by healing long standing lesion that was refractory to all else. Fibronectin powder is concentrated and has great healing powders.

SKIN PROBLEM

Around 25% of patients with AIDS will have skin problem, such as seborrheia dermatitis, pruritus, psoriasis.

This is normally a nutrition problem. Use Omega-3 or Evening Primrose Oil. Pet tab will help with Vitamin A. Garlic for infection. Grapefruit seed extract, a drop or two in water and apply topically, also for infection.

CONSTIPATION

The healthy colon extracts water and electrolytes (such as sodium and potassium) from digested food, Constipation hinders this activity, and leads to dehydration and electrolyte imbalances. These problems are corrected by administering intravenous fluids.

Gastritis can cause constipation. A cat may become constipated because her kidneys are not functioning properly. You may alter her diet to increase fluid intake and decrease the amount of sodium and protein the kidneys must filter from the urine can help prevent dehydration.

Lactulose is normally used. Try stool softener Sodium Docussate (DDS). Herb Black Walnut may help.

Slippery Elm, neutralizes acid in the stomach and intestinal tract. Has a soothing effect on inflamed mucous membranes.

Prune juice acts as a laxative. Aloe vera juice has a healing and cleaning effect and aids in forming soft stools.

Cisapride (Propulsid)..a motility modifier...gastrointestinal stimulant for inadequate stomach emptying.

Sucralfate...prevention of ulcers and irriation.

Ranitidine...gastritis/duodenitis in cats and ulcers of the gastrointestinal tract and esophages caused by kidney failure.

acupuncture...has been used with some success.

DIARRHEA

Infectious diarrhea I found antibiotic Clavamox 125mg one tablet twice daily (every 12 hours) and Grape Oregon for a week to work.

For non-infectious diarrhea a new product came on the market recently from Shaman.

A drug for diarrhea treatment, obtained from a tree in the Amazon rain forest and used for centuries. Results from abbreviated clinical trails suggest it is effective within 4 days for AIDS-related diarrhea (2 days for travelers diarrhea).

You should use also Glutamine which is good for diarrhea, wasting, and raises the level of glutathione.

Help are: garlic, colostrum, acidophilus. Your cat will need vitamin B12 because diarrhea causes a deficiency. Dyhydration is a problem. You can use Gatorade drink for that. Grapefruit Seed Extract, use only a drop or two with water. Another herb is Slippery elm powder, use with some water.

No solid food for the first 24 to 48 hours.

PAIN MANAGEMENT

Pain is common in HIV disease. 50% is relate to HIV infection or the associated opportunistic infections and neoplasms.

To treat inflammation and pain:

FIV NEUROLOGIC DISEASE CAUSING INTERSTITIAL CYSTITIS

Veterinarians have recently noted many similarities between common form of feline lower urinary tract disease and a bladder disorder affecting humans called interstitial cystitis (IC). A psychologically stressful event often precedes the onset of lower urinary tract discomfort due to interstitial cystitis in humans. Some cases of HIV have shown bladder problems related to advancing neurologic disease that cause this condition.

After trying antibiotics several times for tract disease consider that FIV neurological disorder is causing the bladder condition.

Treatment: Folic acid, vitamin B12 and SAMe(S-adenosylmethionine).
