Nurse Waite goes to work, waiting in OR 2 for Dr. Lloyd with the chubby lipo patient. Dr. Lloyd, however, is doing lipo in OR 3, where a man is waiting for scalp reduction. Dr. Lloyd sucks out all the fat and most of the blood from the wrong patient. Nurse Waite notices this -- the ORs have videos and TV outside the rooms -- and tries to stop him. It does no good. The patient is dead.
Credits Roll: The Truth is Out There
Mulder and Scully interrogate Dr. Lloyd at his home in the presence of his attorney. Lloyd tells Mulder and Scully of an almost possessed feeling -- watching himself to something he would never do. This idea works well for Mulder, not so well for Scully. Scully wants to know what meds the Dr. is taking. He is on sleeping pills and stomach medicines. When Scully wants exact dosages, the lawyer steps in.
At Greenwood, Scully finds Dr. Lloyd's medical records and learns he is addicted to his sleeping pills -- a rare pill that can cause behavioral problems. Mulder and Scully check out the operating room. While Scully explains to Mulder how incredibly state of the art this all new OR is, Mulder notices burn marks on the floor. When he connects the burn dots with the victim's blood, they become a pentagram.
Scully, not wanting any part of this, tells Mulder he can play connect the dots all day long, the patient died of medical malpractice because of the doctor's pill problem. As they leave the OR, Mulder notices all the cool videos stuff and wonders how no one noticed the killing. He also has given up the idea of possession in favor of black magic.
Nurse Waite comes out of an OR. Mulder and Scully try to question her but she won't say anything bad about Dr. Lloyd and all but laughs at Mulder's possession/black magic idea. Mulder and Scully try to interview another plastic surgeon -- a female -- Dr. Shannon, but she blows them off.
Several hours later, all the plastic surgeons are meeting with the head plastic surgeon in his office. Head guy Dr. Franklyn, has a very odd design in his conference table -- a pentagram. While all the doctors are furious at Lloyd for leaving them open to investigations, Franklyn seems rather calm. When Dr. Shannon mentions that Nurse Waite was interviewed by Mulder and Scully, he says she knows nothing -- not to worry.
Meanwhile, in OR2, Nurse Waite is prepping another patient for surgery. While the woman is worrying about being knocked out, Nurse Waite has 5 leaches on the patient's stomach. Waite promises to protect her.
At his motel room, Mulder is examining his nose in the dresser mirror. When Scully comes in, Mulder shows her a video of the killing. While Scully is grossed out by the killing, Mulder shows her the pentagram on the floor. The pentagram is for protection, Mulder tells her, proving that blackmagic is involved. Scully is confused, wondering how the dead patient was protected and why this would happen. Mulder said blackmagic works on the vain and weak. Scully wants more proof. Mulder tells Scully that Lloyd's stomach medicine had Bella Donna in it. Bella Donna is a common part of witch's magic potions. The stomach meds are the only medicine which has Bella Donna in it. Scully's response is classic "Lets put out an APB on someone riding a broom with a tall, black hat."
At the hospital, Dr. Shannon is preparing for the chemical peel (the leach lady) while another doctor, Dr. Elanque is cleaning up to leave. When Shannon goes to her OR, the door is locked. Dr. Elanque has taken a laser to the woman's cheek and has burned it away. As Shannon and Nurse Waite try to open the door, the laser has burned through the woman's head.
Scully examines Dr. Elanque who has no memory of what he did. He is on the same stomach medicine as Dr. Lloyd, freaking Scully out. Mulder, meanwhile is in a doctor's office, looking at his nose in plastic surgery software and seeing what he would look like with a smaller, narrower, flatter nose. Scully joins him -- not knowing what he was doing -- and has the video of the surgery. The leeches left a mark in the shape of a pentagram.
While the doctors have another meeting, Mulder and Scully walk in. Dr. Franklyn tells the two about four patient deaths 10 years earlier at the hospital. Nurse Waite was the dead patient's nurse in each of those cases. She left the hospital not long after that and has just transferred back. Mulder and Scully look for her, but she's gone home.
At her home, Nurse Waite performs a black magic ritual -- burning her hair and chanting.
When Mulder and Scully arrive at Nurse Waite's home, she is gone but they find a broom on her doorstep. Mulder jokes that the broom is probable cause but a pentagram carved in her front door has them searching the joint. Scully finds her black magic altar and ritual materials.
That night, Dr. Franklyn returns to his very chic, modern home. His lights do not work. As he looks around in the house, he hears a drip in the bathroom. The bathtub faucet is dripping blood and the bathtub is full of blood. VANITASVANITATUM is written in blood on the bathroom mirror. As Dr. Franklyn looks into the tub, Nurse Waite pops out and tries to stab Dr. Franklyn with an ornate knife. She cuts him and flees.
Franklyn tries to call 911 from a downstairs phone but Waite attacks him again. A well placed punch, however, ends her attack. As Nurse Waite is removed from Franklyn's home by the Chicago police, she is yelling and screaming. She wants to talk to Mulder -- he understands, "its all too strong", she tells him. She then starts vomiting straight pins. Scully goes to the ER with Waite while Mulder remains to question Franklyn.
Dr. Shannon is sewing up the cut on Franklyn's head when Mulder walks in. Mulder makes a quick joke about plastic surgery, but Franklyn isn't laughing. Mulder wants to know how Waite knew Franklyn ratted her out. Shannon answers instead, saying she's nuts. Mulder starts talking about witchcraft which upsets Dr. Franklyn. Franklyn complains he is exhausted so Shannon and Mulder leave. Franklyn dozes, and levitates as he sleeps.
Back in his hotel room, Mulder still obsesses about his nose. An exhausted Scully enters his room, Waite died 20 minutes earlier. Waite died of heavy internal bleeding from vomiting straight pins. Scully starts talking about a disease where people eat weird stuff, but Mulder thinks pins would have killed her going down, not coming up.
Mulder has his own theory. A black magic book he found in Waite's home explains a phenomenon where people vomit vile things including butcher's knives and cue balls. Mulder also found a calendar in the woman's home. The high holy days of the witch's calendar are 1/31, 4/30, 7/31 and 10/31. The dead patients have 4/30 and 7/31 as birthdays He thinks Waite was trying to protect these patients and warn them about the hospital.
The next morning, another doctor fills in for an exhausted Franklyn for another chemical peel. Franklyn's patient has 1/31 as a birthday. The doctor flips and puts too many chemicals on the woman's face, burning it off. Mulder and Scully talk to Dr. Shannon about the 10 year old deaths in the hospital. Four were patients -- killed in negligence accidents while another doctor killed himself with overwork and pills. Mulder demands to see the dead patient's and doctor's files.
The patients b-days were the witch holidays. Dr. Cox's, the dead doctor, picture is loaded into the plastic surgery software that Mulder was fiddling with earlier. Reworking his face, he is morphed into Dr. Franklyn. Meanwhile in his home, Dr. Franklyn is scratching something into his marble floor.
Mulder and Scully go to Franklyn's home to find him. Franklyn, meanwhile is at the hospital. When Dr. Shannon meets him in an operating room, she is nervous -- she did the morphing for Mulder. Franklyn knows she is on to him and is able through black magic to dump all the surgical tools into her stomach without cutting her.
At Franklyn's home, Mulder notices the tile work in the hall is a pentagram. Franklyn has etched all his victims' name into the pentagram, including Shannon's.
At the hospital, Shannon is being prepped for emergency surgery, but she wants to tell them what happened to her. She can't, too weak. Franklyn anesthetize her and leaves the OR. In a neighboring OR, Franklyn starts to operate on his own face -- no anaesthesia. While Scully tries to stop the Doctors from accidentally killing Shannon, Mulder looks for Franklyn.
Mulder finds Franklyn's face and face only, on the floor of the OR. He is sickened by it. Scully is able to save Shannon but no one can understand how she swallowed all those tools.
In another OR, a doctor flips and kills his patient. That patient's birthday is 10/31 -- she is the final death.
Several days later, at City of Angels hospital in LA, a new good looking doctor joins the plastic surgery team. He is a younger version of Franklyn.
